
Vitamin D Testing: What 25-Hydroxyvitamin D Actually Measures
Why labs measure 25(OH)D rather than the active hormone, how deficiency thresholds are set, and why Indian reference ranges are debated.
Vitamin D is unusual among vitamins: the body manufactures most of it in skin exposed to ultraviolet B light rather than obtaining it from food. It is also not really a vitamin in the classical sense — once activated it behaves as a hormone.
Why labs measure 25(OH)D and not the active form
Vitamin D is activated in two steps. The liver converts it to 25-hydroxyvitamin D, and the kidneys convert that into 1,25-dihydroxyvitamin D, the biologically active hormone. Intuitively you might expect the test to measure the active form. It does not, and for good reason.
- 25(OH)D circulates at far higher concentrations and has a half-life of two to three weeks, so it reflects accumulated status rather than the last few hours
- The active form is tightly regulated by parathyroid hormone, and the body will hold it near-normal by drawing on stores — meaning it can look fine while stores are depleted
A report headed "Vitamin D (Total)" or "25-OH Vitamin D" is measuring the storage form. That is the intended test, not a substitution.
How thresholds are set — and disputed
Vitamin D cut-offs are not derived the way most reference ranges are. Rather than taking the middle 95% of a healthy population, they are set against biological endpoints — chiefly the level at which parathyroid hormone stops rising and calcium absorption plateaus.
Different expert bodies have drawn the line in different places, which is why two laboratories may label the same value differently. The Endocrine Society and the US Institute of Medicine have published notably different thresholds for sufficiency, and neither is a fringe position.
There is an additional wrinkle in India. Reference ranges derived from largely European populations may not transfer cleanly to populations with darker skin pigmentation, different sun exposure patterns, and different dietary calcium intake. Whether Indian sufficiency thresholds should differ remains an active question in the literature rather than a settled one.
Units, and a factor-of-2.5 trap
Vitamin D is reported as ng/mL in India and much of the US, and as nmol/L in Europe and in many research papers. The conversion is 1 ng/mL = 2.5 nmol/L. A value of 30 ng/mL and one of 75 nmol/L are the same result. Comparing across the two units without converting produces a dramatic and entirely artificial difference.
Why it is often ordered with calcium
Vitamin D's main established role is regulating calcium absorption from the gut and calcium balance in bone. Calcium, phosphate, and sometimes parathyroid hormone are frequently measured alongside it because the panel describes one system — the vitamin D level alone does not show what that system is doing.
